NURSES - RN'S, LPN'S, CASE MANAGERS
They provide nursing care in accordance with the patient's plan of care to include comprehensive health and psychosocial evaluation, monitoring of the patient’s condition, health promotion and prevention coordination of services, teaching and training activities and direct nursing care.
They offer patients one-on-one therapy and treatment that is high-quality, personalized and effective. They collaborate with a team of healthcare professionals to provide a treatment plan, solutions for independence, progress monitoring, documentation and assessment and assistant and aide supervision.
They enact an all-encompassing care program that includes rehabilitative assessment and evaluation, muscle and mobility assessment, caregiver instruction, physical therapy regimen, patient education and much more. Their ultimate goal is to get you back in fighting shape and feeling great.
They tackle a variety of tasks under the supervising of a nurse, including bathing and grooming duties, food preparation, housekeeping, patient mobility, toileting, transportation and health monitoring. One of the most important roles that a non-medical caregiver plays, though, is that of companionship – an all-day presence to keep each patient company.
MEDICAL SOCIAL WORKERS
They provide medical social services to patients in their homes in accordance with physician orders and under the direction and supervision of the Director of Nursing or another appropriate supervisor. They assist in the admission process of the patient to Prosperous Home Health by performing an initial evaluation, assessing the patient’s psychosocial status and evaluating the patient, family and home to identify socioeconomic and emotional factors that will affect the plan of treatment.
